I use this activity with my low English ability agricultural HS students. Before handing out the worksheet I first give a brief explanation about Yes-no questions and when you use "do you" vs "are you" and show some examples of how a basic sentence is transformed into a Y-N question. I then do this with the entire class with the first two sentences on the worksheet. Students are to write the question form on the right hand side of the sheet, under "Questions". I give the class a few minutes (or more depending on their level) to form a Y-N question for each sentence. Then as a class we go over them and they correct their errors (can also ask Ss who finish early to write some of them on the board).
Now I ask all students to stand and ask their classmates these questions, and find two people to answer yes for each question. If its a large class I tell them they can only have each person's name once to avoid them asking one person all the questions.
