This worksheet is designed for students to practise forming "have you ever...?" questions in the present perfect e.g. Have you ever seen a ghost? You can brainstorm a list of words on the board before giving out the sheet to encourage students to use a variety of verbs to create interesting questions.
Students think of their own questions and write them in the boxes. Then they go around the classroom asking each other their questions and recording Y/N for their friends' answers. The 'extra info' box is there to encourage follow-up questions (e.g. Have you ever seen a ghost? -> Yes, I have. -> Where did you see it?).
After students have talked to everyone else in the class, review the 3rd person singular form "John has seen a ghost / John has never seen a ghost" and get each student to share something interesting he/she learned about at least one person in the class.
