This worksheet copies the format of one uploaded by another ISLCollective user PhillipR, but with new sentences to be used if students need more practice for when to use the words Am,Is.Are, Was, Were, Do, Does, and Did. This is an excellent practice activity that helps students learn when to use the Present Simple and Past Simple as well as the correct form of "to be" or the helping verb "do." Hand out the Student A and Student B worksheets to each student in your class and have them fill in the correct word, after checking their answers, have Student A pair with Student B and have them ask each other the questions, allowing for good conversation between the students.

The above downloadable handout is intended for at Elementary (A1) and Pre-intermediate (A2) level. It is a useful material for practising Past simple tense, Present simple tense and Questions (interrogative) in English, and is excellent for enhancing your groups' Speaking skills.
